AGMC Services provides professional Alcohol and Drug Assessments conducted by certified counselors using evidence-based tools. These assessments deliver a clear, objective evaluation of substance use patterns and related life areas, meeting the requirements of courts, employers, and treatment programs. Each assessment includes a confidential interview and a detailed written report outlining the results and recommendations.

We offer the following assessment options:

• Alcohol and Drug Assessment (ASI – English): A full evaluation using the Addiction Severity Index (ASI) tool, conducted in English by a certified counselor.

• Alcohol and Drug Assessment (ASI – Spanish): The same comprehensive assessment, administered in Spanish for native speakers.

• Updated ASI Assessment: For individuals who have previously completed an ASI assessment and need to update their results due to changes in circumstances, treatment progress, or reporting requirements.

• Collateral Reference: Required for some assessments, this form allows a third party (such as a family member, employer, or treatment provider) to provide supporting information that enhances accuracy and completeness.
